padi Medical questionnaire faq's

OK so you have decided to complete a PADI diving course with us. We will now ask you to complete the standard PADI Medical Questionnaire (Located here). The purpose of this form is to capture important information about your general health with particular reference to

Any existing medical issues requiring treatment?
Any existing drug use?
Do you have or have had any medical issues with your cardiovascular or respiratory systems?

The instructions on the form simply ask you to write a 'Yes' or a 'No' next to each question. You must enter 'No' for everything to be able to learn to dive without further medical examination.

All is not lost if you enter 'Yes' to any question. You simply have to go and see a diving doctor and get signed off for the particular problem. Depending upon what you answered 'Yes' to the doctor may have to do simple tests or more complex ones.

Please note that customers with diabetes are able to learn to dive with Infinity as long as the condition is well controlled. We have a lot of experience in this area so please email us with any questions that you may have.
